I (a) give notice that I apply for an order for the recovery of the sum of pounds shillings and pence, due in respect of the tithe rent charge issuing out of the lands described in the first part of the schedule to this application for the period of (b) and which became payable on the day of 189 The owner of the said lands is (c) thereof [or I do not know or cannot ascertain the owner of the lands]. and the owner is himself the occupier I shall apply that the order for recovery be executed by the appointment of an officer to distrain under sub-section (2) of section 2 of the Act.
